Inquest date changed regarding 2014 construction fatality

BARRIE, Ont. – A mandatory inquest into the Feb. 6, 2014 death of construction worker Steven Mitchell has been scheduled to begin on May 15 at the Forensic Sciences and Coroners Complex in Toronto, according to an announcement from Dr. William J. Lucas, the regional supervising coroner for Ontario’s west region in the central west office. A March 28 news release from the Ontario Ministry of Community Safety and Correctional Services stated that the inquest had originally been scheduled to start on May 17, but no reason for the date change was given. Dr. Maarthen Reinders will preside as the coroner, with Raymond Williams as counsel to Dr. Reinders at the inquest, which is expected to take three days and hear from five witnesses. Mitchell, an employee of M. Fuda Contracting, was helping a colleague dismantle trench boxes from an excavation site in Barrie when a box shifted and caused him to fall off a ladder, causing head injuries. After treatment at two local hospitals, Mitchell was sent to Toronto’s St. Michael’s Hospital, where he passed away. Last year, M. Fuda was fined $60,000 for its role in the incident (COHSN, Sept. 27). Mitchell was 53 years old at the time of the accident.
